1. Health Declaration Form (see also in appendix) is required for essential trips to China. Please download the form from the embassy website and fill it truthfully. Carry it with you all the time for possible check. Entry might be hindered should you have concealment or false information. Legal liability might be charged if transmission or the risk of transmission of COVID-19 is hence incurred.

2. A 14-day quarantine is required upon entry. Expense to be covered by the traveler.
